- Your referral, if required
- A list of current medications and supplements
- Relevant laboratory or imaging results
- Previous endoscopy or colonoscopy reports
- A list of allergies
- Your questions and concerns
Insurance and Payment
Please contact the office before your visit to confirm:
- Whether your insurance plan is accepted
- Whether a referral or pre-authorization is required
- Your expected co-payment
- Which services may require separate payment

Cancellation
We understand that plans can change. If you cannot attend your appointment, please contact us as early as possible so that the time may be offered to another patient.
